Practices have unusual phone needs: hours that vary by day, on-call rotations, after-hours messages that need to reach a real provider in emergencies, and a regulatory environment that's careful about what data lives where. Kelir delivers the same UCaaS feature surface as RingCentral, Dialpad, or Spruce — auto-attendant, multi-level IVR for tier-routing, hunt groups for on-call rotations, departments per provider, voicemail with transcripts, recording with configurable retention, working hours per day — on a real cellular eSIM line per seat. Voicemail audio + transcripts, recordings, and routing config all stay in AWS US data centers. HIPAA-aware: architecture is consistent with HIPAA-grade controls (encryption in transit/at rest, access logging, retention up to 7 years on Pro), but the product isn't currently sold under a customer-facing Business Associate Agreement — BAA is on roadmap for Team Pro. Working-hours routing per day

Different hours Monday through Saturday, plus holiday closures. After-hours calls route to a covering-provider line, an emergency voicemail with custom greeting, or a paging service number — configurable in the portal.

On-call rotation hunt groups

Group practices with rotating on-call duties: Team Starter hunt groups route after-hours calls to whichever provider is on. Reorder the group from the portal Sunday night for the week ahead.

Voicemail transcripts on the lockscreen

Patient leaves a message after hours; you see the transcript on your lockscreen and decide whether to return the call tonight or tomorrow. No app to open, no PHI screenshot risk.

Front-desk-friendly portal

Web portal at app.kelir.io for the practice manager: change hours during a holiday, update the after-hours greeting before a snow day, add a new staff member to the hunt group.

Group practices with 2–10 providers want Team Starter — hunt groups for on-call rotation, departments for separate provider lines, working-hours per provider, and a portal the practice manager can run. Move to Team Pro ($39.99/seat/mo) if you want call analytics or want to negotiate a BAA.

Who it's not for: If your practice requires a fully HIPAA-compliant phone system under a signed BAA today, Kelir's HIPAA story is roadmap-stage rather than shipped — engage sales before committing. Larger practices (15+ providers) often want a healthcare-specific platform like Spruce or OhMD that includes secure messaging by design.

Common questions

  • The architecture is HIPAA-aware (encryption in transit/at rest, access logs, retention controls, US data residency, AWS Business Associate Addendum on the underlying infrastructure). Kelir does not currently offer a signed Business Associate Agreement (BAA) with customers — that's on the roadmap for Team Pro. Use Kelir for non-PHI practice phone use cases today; engage sales if a BAA is required before launch.

  • Voicemail audio and transcripts are stored encrypted in AWS US-East-1. Until a BAA is in place, customers should treat Kelir voicemail like any non-BAA voice channel — appropriate for appointment scheduling and basic logistics, but not for unsolicited PHI exchange. Greeting options include explicit "please don't share medical details on this voicemail" prompts.

  • Create a hunt group with the on-call providers, set the routing strategy (ordered cascade is most common — primary on-call first, secondary if no answer in 25 seconds, etc.), and update the order weekly from the portal. After-hours calls route through the hunt group; daytime calls bypass it and ring the front desk.

  • Yes — Departments on Team plans give each provider (or each office) its own US business number, working hours, voicemail box, and routing rules. Front desk has the main number; each provider has a direct line for established patients.

  • Kelir supports A2P 10DLC SMS for appointment reminders. The pre-registration covers transactional messaging like reminders and confirmations. We don't currently offer a healthcare-specific automation platform — practices typically pair Kelir for the voice line with a telehealth platform (SimplePractice, Doxy.me, etc.) for visit-side workflows.
